I got a renewal pack in the mail stating, "Complete and return the enclosed renewal application immediately to help assure the div. of licensing has a sufficient time to process your renewal prior to the expiration date of your current license."
You'll need the renewal form, signed and notorized, a passport-type photo, and a check for $65
