I am looking for help from any Monroe County Instructors.
The latest Pistol Permit on their web site has a HIPAA Release form that every applicant must sign.
It has the following paragraph:

In the event my application is disapproved, the sources of any confidential information will not be revealed to me. I agree to indemnify and hold harmless the organization and the person to whom this request is presented as well as their agents and employees from and against all claims, damages losses and request expenses including reasonable attorney fees arising out of or by reason of replying with this request.

Can anyone tell me when that was added? This is a "Hold Harmless" agreement. It totally makes the applicant voluntarily give up all of his constitutional rights just to get the permit processed.

Is this recent? It does not exist in Erie county.......................yet but Monroe is like a lap dog for Cuomo.

I know but this appears to be new you can see it at Link Removed
It says
If you take any medication for anxiety, depression, bi-polar disorder, post traumatic stress disorder, etc or have been to counseling of seen a Psychiatrist or Psychologist for any reason, You must compete the attached Authorization for Release of Personal Information. This wil prevent any further delays in processing your application. (Being on medication or receiving mental Health services is NOT an automatic dismissal)

Then is says: NOTE: Please leave the expiration date blank, The investigating deputy will fill it in upon contacting your prescribing MD or counselor (See Form 4, Item #3)

Then at the end of Form 4 there is a very disturbing paragraph
