Question - Mental Health Release Form

Howdy -

I am preparing my concealed permit application and have a question regarding the "Mental Health Release Form:"

The main section of this form is for disclosing previous health providers. Question: If I have never used a health provider, do I simply leave this section blank?

In other words, is it sufficient to fill out the top and bottom portions, but to not fill out any information in the "Name of Provider" section, since to provider exists?

I put down NONE due to the fact we just had relocated into the State. Not an issue on my issuance by my Sheriff.
