Need advice

I am new to forum, so hello to all. Maybe 10 years ago I had a Indiana CC permit that I let expire plus I no longer have that old permit. I have decided to apply for a new permit, Indiana has a on-line request forum that can be filled out to apply, there is question on the form asking if I have ever held a Indiana CC permit in the past and also asks for the id number and year applied for. This stops me from completing the application due no having that info. Does anyone know if and how I could get the info on my old permit helded?

Also Live in Indiana and had almost exactally same situation. I went to the police station and had them fill out the form (also online). She just skipped the info on previous permit. They can do that, I didn't want to when I tried online. Good luck.
