NC Mental Health Release Form Question

Hi, I'm about to drop my application to the Sheriff in my county, but NC requires a 'Mental Health Release' form to be filled out (Link Removed.

My question is, I have never been to a Dr for any mental health issues and I haven't been to a primary care physician in years. the only thing that I even remeber who/what/when was a dentist about a year ago. Is it not uncommon to just sign this document and leave it blank? Or do I have to try to find doctors names in walk in clinics from when I was a teenager and childhood doctors?

Why don't you fill out the info you have for the Dentist? He is a Medical Doctor, and he can look back in your chart and verify you didn't have any obvious mental health issues...
