I got this on Facebook and just had to share it. I, like the sender, have to fill this out! :lol:

LIVING WILL FORM

I, ______________, being of sound mind and body, do not wish to be kept alive indefinitely by artificial means.. Under no circumstances should my fate be put in the hands of pinhead partisan politicians who couldn't pass ninth-grade biology, if their lives depended on it, or lawyers/doctors/hospitals interested in simply running up the bills.
If a reasonable amount of time passes, and I fail to ask for at least one of the following:
______ a Martini ______ a Margarita ____ a Scotch and soda ______ a Bloody Mary ______ a Gin and Tonic _______ a Tee Time ______ a Steak ______ Lobster or crab legs ______ the remote control ______ a bowl of ice cream ______ the sports page______ sex ______ or chocolate: It should be presumed that I won't ever get any better. When such a determination is reached, I hereby instruct my appointed person and attending physicians to pull the plug, reel in the tubes, and call it a day. At this point, it is time to call the New Orleans Jazz Funeral Band to come do their thing at my funeral and ask all of my friends to raise their glasses and remember all the good times we have had.

NOTE: I also hear that in Ireland they have a Nursing Home with a Pub.. The patients are happier and they have a lot more visitors. Some of them don't even need embalming when their time comes.
If anyone knows the name of this happy place, PLEASE send me the data so I can pre-register.

And it would be heavenly if Glock made a single malt scotch. What a way to go!!!
 
On a serious note we knew that my mother was starting to have mental problems. We talked her into having a Durable Power of Attorney drawn up naming one of her children as her Power of Attorney. Not long after we had to move her to an assisted living center and later to a nursing home. The POA made things much simpler for us with all the decisions that we had to make and things we had to do such as with her finances etc. If we had not had it drawn up beforehand we would have had to go to a judge and get him to appoint someone to do all of that. So think about this for yourself or your parents (or other loved ones) and get something drawn up ahead of time. The lawyer who did the work for us told me that if we had waited just a couple of months he would not have done is as she was then to the point that she was not of sound mind.

As for the living will let those who will take care of you know your wishes. It worked out in the end but I later learned that what I thought and what my Father wanted were two totally different things.
