I like the artwork but I'm confused how a shirt bragging about gun size is a fundraiser for cancer. Maybe I don't see the connection.

Well, since the OP hasn't answered yet I'll take a shot at it. I don't think that it's explicitly supposed to be about cancer, rather that the shirt would generate fans/buyers and the proceeds would then go to a cancer charity/research center. That's what I think...
